Job Description: Full-Time Targeted Case Management Position for Substance Use Disorder Clients
Position Overview:
We are seeking a dedicated and compassionate individual to join our team as a full-time Targeted Case Manager (TCM) specializing in substance use disorder clients.
As a TCM, you will provide comprehensive support, advocacy, and coordination of services to individuals struggling with substance abuse in accordance with the Kentucky targeted case management regulations.
Your role will involve conducting assessments, developing individualized care plans, and linking clients to appropriate resources and services.

Responsibilities:
1.
Conduct initial and ongoing assessments to determine the needs, strengths, and preferences of substance use disorder clients.
2.
Develop and implement individualized care plans based on the assessment findings and in compliance with Kentucky targeted case management regulations.
3.
Provide ongoing case management services to clients, including monitoring progress, advocating for clients' needs, and ensuring the coordination and continuity of care.
4.
Collaborate with clients, their families, and relevant service providers to establish and maintain effective working relationships.
5.
Link clients to appropriate community resources, including treatment programs, healthcare providers, housing assistance, vocational services, and other supportive services.
6.
Assist clients in accessing and navigating the healthcare system, ensuring timely and appropriate medical, mental health, and substance abuse services.
7.
Facilitate referrals and coordinate communication between various service providers to promote a seamless delivery of services.
8.
Provide crisis intervention and support to clients during times of acute need, ensuring their safety and well-being.
9.
Maintain accurate and up-to-date documentation of all case management activities, including assessments, care plans, progress notes, and other required documentation.
10.
Adhere to all confidentiality and privacy regulations in handling sensitive client information.
11.
Stay informed about current trends, best practices, and regulations related to substance use disorders and targeted case management in Kentucky through ongoing professional development and training.
12.
Participate in team meetings, case conferences, and supervision sessions to enhance knowledge sharing, collaboration, and quality improvement efforts.
Qualifications:
1.
Bachelor's degree Required
2.
Minimum of two years of experience in case management or related field, preferably with substance use disorder clients.
3.
Strong understanding of Kentucky targeted case management regulations, policies, and procedures related to substance use disorders.
4.
Knowledge of local community resources, treatment options, and support services available to individuals struggling with substance abuse.
5.
Excellent communication and interpersonal skills to establish rapport and effectively engage with diverse populations.
6.
Ability to assess clients' needs, develop comprehensive care plans, and monitor progress towards goals.
7.
Proficiency in documentation and record-keeping, ensuring accuracy, timeliness, and compliance with regulatory requirements.
8.
Strong problem-solving and organizational skills, with the ability to manage multiple cases and prioritize tasks effectively.
9.
Empathy, compassion, and a non-judgmental attitude towards individuals affected by substance use disorders.
10.
Ability to work both independently and collaboratively within a team environment.
11.
Knowledge of crisis intervention techniques and the ability to respond appropriately to emergency situations.
12.
Valid driver's license and access to reliable transportation to travel to client locations and community resources as required.
