POSITION SUMMARY: The HIFI Facilitator promotes youth and family development through integration of natural support systems, development of skills to manage and sustain the child living in the home.
QUALIFICATIONS: A bachelor's degree in a human services related field, and one year of experience providing TCM services to children/youth with SED and their families.
Record of certification from a state recognized High 'Fidelity Wrap-Around Core training.
SKILL SETS: Adequate communication skills, process verbal and written instructions, dependable, interact effectively with the public and staff, adaptability-able to deal with frequent change, delays or unexpected events, strong sense of commitment.
Facilitator must have a collaborative personality that leads to trust and the engagement with others and an ability to understand the point of view of others.
The ability to manage details and to follow-through until goals are completed.
Facilitators need to have a culturally competent acceptance of diverse individuals and their families, and a willingness to be part of a team and ability to work effectively in a team environment.
PHYSICAL AND ENVIORMENTAL DEMANDS: Frequent and hearing when communicating with children/families and the collaborating professionals.
Possible exposure to communicable diseases and unsanitary conditions during daily home visits and during activities listed above.
Must be able to kneel, push and lift.
Daily driving of automobile requires Visual acuity, physical coordination and accurate depth perception.
JOB SPECIFIC FUNCTIONS: The HFW Facilitator assures that care is delivered in a manner that respects family culture, supports "voice and choice" protocol, and adheres to the Department for Behavioral Health's "strengths-based" focus throughout the process.
Offers consultation and education to all providers regarding the values of the wraparound process, monitors progress toward goals, and assures that all necessary data for evaluation are gathered and entered.
Responsible for completing a comprehensive strengths-based assessment of the individual and family, working in full partnership with team members to develop and implement a plan of care, identifying formal and natural supports, and facilitating team meetings.
Provide regular home visits and monitoring implementation of the team vision, mission, and plan of care.

Maintains ongoing contact with schools, therapists, and other professionals for each child.
Communicates with team members regularly regarding child's needs, goals, functioning level, and discharge plans.
Conducts complete assessment of child's functioning, current family situation, risk of out-of-home placement and identify underlying needs for addressing crisis and out of home placements Develops a service team for each child on their caseload and facilitates service team meetings to develop an individualized, plan of care, monitors and implements the plan, and serves as the liaison to all members between meetings.
Assists the family in understanding the service team/wraparound process, information shared, and recommendations for locating additional services and/or resources.
Facilitates service team meetings to address child/family needs and.
problems to the Service Team and makes required face-to-face contact with child, parent/guardian, and other service providers.
Works actively to resolve problems or conflicts occurring regarding child.
Facilitates appointment with family.
Maintains essential medical records and documentation.
Responds by phone in a timely manner to crisis calls and advises caller of possible options for resolution of the crisis.
Arranges for transportation for child and family as necessary, Assists with program planning, development and evaluation.
Maintains ongoing contact with the child's placement facility if child is removed from family/community placement.
Disseminates information regarding the High Fidelity Wraparound program and its focus, promotes awareness and cooperation of persons becoming involved with High Fidelity Wraparound, and assists other professionals in developing a more thorough knowledge base about the needs and treatment of children with serious emotional disturbances.
Reports any high risk, physical management, or exposure incidents immediately to supervisor.
Participates in, agency continuing quality improvement (CQI) activities.
Willingness and ability to complete I-IFW Facilitator training An understanding of and experience with different child-serving systems, including education, behavioral health, child welfare, juvenile justice, health and others Experience working with and genuine respect for families of children/youth who have complex behavioral health needs and a firm commitment to empowering families An extensive knowledge of community resources Access to reliable transportation and a valid driver's license Must be available during regular work hours as well as some evenings and weekends Maintains fiscal accountability.
Performs other duties as assigned.
