POSITION SUMMARY: Assists ACT Manager with overall operations of Assertive Community Treatment 24/7 team, coordination with other administrative and clinical programs.
Ensures consumer care and fidelity to evidence based practice.
Coordination with community partners and managed care organizations.
QUALIFICATIONS: Minimum Bachelor's degree and 1 year experience in related field.
SKILL SETS: Demonstrates superior oral and written communication skills for effective customer service/community relations.
Possesses an advanced knowledge of Microsoft Office programs including gathering and compiling data to prepare reports.
Other demonstrated skills necessary include ability to be calm and attentive, problem analysis/ resolution, organization, dependability and adaptability.
PHYSICAL AND ENVIORMENTAL DEMANDS: Verbal and receptive abilities required to communicate with clients, family members, community partners and other staff.
Visual acuity and manual dexterity is required for computer programs and written communications.
Work is light with some walking, standing, and sitting.
JOB SPECIFIC FUNCTIONS: Receives and disseminates all crisis calls during the working day for the ACT Team and consults with supervisory chain for critical situations.
Reconciles all deposits into operating account weekly for ACT bank account.
Provide support and assistance to conduct referral screenings, admissions and discharges.
Assist with maintaining proper documentation of clients' medical records, including but not limited to consents, and other administrative documentation the correction process.
Assist with the development and implementation of new program ideas to create an involving and progressive therapeutic treatment environment.
Provides assistance to ensure the accurate and timely submission of required reports, and documentation to various state and local agencies.
Disseminate monthly client list to various external supervisors and ACT staff.
Track and chart all psychiatrist/ PA sessions to ensure compliance with fidelity Provide administrative support to the ACT drop-in center Reviews correspondence for ACT staff and supervisor signatures Receives telephone calls and visitors, and arranges for appointments.
Establishes and maintains Credible Electronic Record system compliance for ACT clients Maintains current knowledge of ACT policies and procedures.
Records minutes from Daily ACT treatment team meetings and disseminates Conducts supply inventory and orders needed program supplies.
Enters updated, admission, financial, and terminated information into the computer Maintains ACT program calendar to include scheduling for staff, clients and collateral meetings.
Coordinates interface of ACT with the community partners to include Hardin Memorial Life spring Unit, Detentions Centers, Primary Care, Guardianship, Alternative Sentencing, and Central State Hospital etc.
Keep a tracking log for all ACT Team referrals to ensure triage into program based on team recommendations along with acceptance or denials.
Collects information regarding maintenance/safety needs of the ACT program and notifies designated staff Consults with ACT Program director or designee for critical situations Track managed care authorizations and denials on a monthly basis Performs other duties as assigned.
