Pre-Employment Requirements:
Drug test
Criminal Background check – over three years since prior conviction
TB skin test: Must have a test or provide proof of one within the past year.
Must have a valid driver's license, reliable transportation and current automobile insurance.
Must have a minimum of a Master’s Degree in a human service-related field.
Must be licensed to provide substance abuse and/or mental health services in the state of Kentucky.
At least 2 years’ experience in providing substance use disorder services.
Position Summary: Access, coordinate and monitor services to assist the client in becoming stable in the community and reaching the highest level of potential with alcoholism or substance abuse.
Skill Sets: Demonstrate good oral and written communications.
Maintain a supportive and encouraging working relationship with co-workers and clients.
Participate in the initiation, coordination and implementation of the referral plan.
Follow-through, monitor and evaluate activities related to the recovery of the client.
Must have basic computer skills.
Physical and Environmental Demands: Visual acuity and manual dexterity are needed for required paperwork.
Frequent talking and hearing are required when communicating with co-workers, families, and other service providers.
Physical demands are light with walking, standing, and occasional stooping.
Possible exposure to communicable diseases during contact with individuals released from various locations i.
e.
jails, hospitals, unsanitary conditions, rehabilitation centers, etc.
Job Responsibilities:
Provide quality customer service to personnel, persons served, families and stakeholders.
Help maintain the health, safety and welfare of personnel, person served and stakeholders.
Ensure the agency is operating under CARF accreditation standards.
Ensure the company adheres to the state and federal licensure and regulations.
Conduct client intakes, screenings and referral services.
Provide individual and group services for clients with substance abuse disorders and a variety of presenting problems in a Residential, Intensive Outpatient and Outpatient setting.
Participate in regular treatment team meetings.
Prepare treatment plans, write SMART goals and objectives
Complete initial and concurrent ASAM reviews.
Complete all clinical documentation in accordance with agency documentation timeframe policy.
Work as part of a multi-discipline team with emphasis on brief, goal-oriented therapy.
Consult and educate in the community and with agencies such as law enforcement, industry community-based services.
Clinicians may be required to work an altered schedule and provide clinical services one to two evenings per week.
May be required to work a rotating, weekend, on-call schedule.
Build a curriculum and provide psych education class/process groups.
Client Advocacy
In collaboration with other staff and case managers, initiate and maintain complete up-to-date client records including but not limited to client files, incident reports, and discharge plans.
Collaborate with other Program Managers and facility staff in program development and client consultation.
Provides counselling, feedback, and education to all clients in collaboration with other members of Leadership team.
