JOB SUMMARY
Responsible for the delivery of patient care that promotes safety and well-being of all patients in the assigned departments.
Plans, coordinates and evaluates overall clinical nursing practice and management activities in the department.
Participates in the development and implementation of hospital policies and procedures.
Maintains open communication with all staff to promote improvement in performance.
This position requires a sound knowledge of nursing practice and management skills.
Job Requirements JOB REQUIREMENTS Minimum Education Licensed as Registered Nurse in the state of KY.
Minimum Work Experience Minimum of 3 years nursing experience in area of practice.
One year of supervisory experience.
Required Licenses [Kentucky, United States] Registered Nurses Current Kentucky RN license.
Required Skills Must be able to follow directions and to perform work according to department standards when no directions are given.

Demonstrates competency and knowledge in the proper technique for the administration of medication for the patient population served.
Demonstrates knowledge of appropriate technique to obtain vital signs and is able to interpret norms for the patient population served.
Demonstrates knowledge of location and use of emergency equipment and drugs for the patient population served.
Provides education to patient/family based on identified learning needs of the patient population served.
Essential Job Functions Clinical Care: Performs or oversees the collection of holistic assessment and development and implementation of a plan of care.
Assesses the patient population daily.
Makes rounds with physicians and care givers daily to discuss patient needs.
Provides direct patient care when appropriate to the acuity of the patient.
Leads and participates with the interdisciplinary team in planning delivery and evaluation of patient focused care.
Interacts on a continuous basis with other disciplines to resolve issues as they arise.
Trouble shoots complex problems in patient care, documents findings and progress in patient record.
Serves as a resource person for new care providers and reviews their progress in the provision of patient care.
Meets with orientees regularly to monitor their progress and maintains written documentation of progress, including the skills check sheets.
Provides feedback as necessary to individual staff members regarding performance.
Oversees patient/family preparation for care throughout the facility.
Ensures physicians are notified of pertinent information regarding their patients.
Outcomes Manager: Reviews patient information, including lab and other test results and progress notes in patient health record daily.
Reviews physician orders and notes and or speaks directly with physicians to discuss patient care issues.
Assesses patient’s response to treatment and care through other care provider information or by examining patients directly.
Collects data on patient outcomes on the unit i.
e.
pressure ulcers, medication errors, falls and etc.
, looking for trends that indicate the need for intervention to improve outcomes.
Serves as a member of the Performance Improvement Committee.
Implements Performance Improvement activities as deemed necessary by patient outcomes.
Uses evidence based practice to evaluate effectiveness of patient care.
Writes or revises unit based procedures/policies/protocols to reflect latest evidence at least annually.
Patient Advocate: Develops a partnership with care providers, patients and patient’s families.
Ensures that patient goals are set with patient and family involvement.
Ensures patients have access to the resources needed to provide optimum care.
Collaborates with other disciplines regarding available options for patient care while in the hospital and after discharge.
Ensures patient or patient’s family is educated on the options for care needed by the patient.
Facilitates care provider discussions including the patient and family with ethical dilemmas arise.
Promptly & fully investigates & responds to complaints of patients, visitors or patient’s families either personally or by appropriate referral.
Educator: Patient/Family Specific: Provides or oversees the education of the patient and family to understand illness and self care concepts.
Utilizes the most current tools to access information for teaching.
Ensures the appropriate assessment of learner needs.
Creates or coordinates the development of patient education materials.
Serves as a member on the Patient Education Committee.
Care Giver Specific: Ensures staff receives proper orientation prior to assuming patient care responsibilities.
Provides opportunities for less experienced nursing staff to observe and participate in physical assessment, data review and individual outcomes.
Assures staff is engaged in clinical rounds as required by policy.
Creates or coordinates the development of evidenced based education to enhance knowledge of clinical care givers.
Assists with providing both formal and informal staff education through inservice/staff meetings on a quarterly basis to enhance nursing skills.
Confers with affiliated schools of nursing in evaluation of the appropriateness of clinical experiences for nursing students.
Promotes opportunities for professional growth and development of staff.
Provides input to nursing leadership on the evaluation and competency of nursing staff.
Develops and maintains open lines of communication and timely transmission of information.
Assists with evaluation and development of information technology tools.
Assures all staff is trained on proper use of information technology.
Monitors confidentiality of patient information and addresses breaches when necessary.
Promotes a positive work environment which enhances employee and patient satisfaction.
On a daily basis delegates, supervises and evaluates the nursing care given by others while retaining accountability for the quality of care given on the unit.
Participates in the interviewing, hiring and termination of employees.
Ensures staff completes all mandatory inservice education requirements as evidenced by education records.
Performs employee performance appraisals 2 weeks prior to the due date.
Monitors staff absenteeism and tardiness on a monthly basis and takes corrective action when needed.
Employs progressive discipline as described in policy for attendance problems and documents actions.
Appropriately completes staff schedules in a timely manner, adhering to acceptable staffing ratios.
Assists with development and justification of budget on an annual basis.
Continually evaluates the morale of staff and takes appropriate actions to resolve problems.
Demonstrates an ability to identify and resolve problems, uses initiative and good judgment to reach quality decisions.
Conducts all aspects of supervision in a firm, fair and consistent manner as evidenced by employee files.
Participates in at least 75% of staff meetings as evidenced by attendance records.
Is punctual in attendance to meetings.
Maintains calm and effective behavior during stressful situations.
Completes assignments in a timely manner and willingly assumes additional responsibilities as assigned by nursing management.
