Also, care is given by registered nurses 24 hours a day.

Job Summary: The Stepdown / Referral Navigator is responsible for the overall integrity of the stepdown process.
As Stepdown Navigator this individual will be the point person for developing strategies to identify those eligible to stepdown to outpatient programming.
This individual will attend daily morning meetings and work with social workers to determine who is eligible for LTBHS outpatient programming.
This individual will also meet with patients to discuss the benefits of outpatient programs, have patient sign all necessary paperwork to start outpatient programming, input data into the Midas system, report trends to Leadership, run reports and manage stepdown workflow.
The Stepdown Navigator will attend all outpatient meetings that pertain to outcome measures and outpatient census.
This person will also communicate with outside referral sources with thank you letters to include disposition, therapist and physician names, and a treatment team date and time 24 to 48 hours after admission.
Education/Experience: Master's degree in human services/clinical area Or RN
