Registered Dietitian Position Summary: The Registered Dietitian screens and assesses participant nutritional status.
Uses pertinent data to plan and implement appropriate nutrition interventions and communicates the information to the Interdisciplinary Team to ensure the nutritional needs of the participants are met.
Performs nutritional assessment and provides nutritional education for the participants requiring interventions.
Coordinates serving of meals; monitors/coordinates kitchen facilities to ensure standards are met.
Coordinates with contracted meal services to ensure meals meet the needs of the PACE participants.
Provides education to other health care professionals as appropriate.
Registered Dietitian Position Duties & Responsibilities: • Conducts an initial comprehensive nutritional assessment on all PACE participants upon enrollment and an in-person reassessment minimally on a bi-annual basis or as needed.
• Participates as a member of the Interdisciplinary Team (IDT) by developing initial and appropriate participant Plan of Care (POC) for nutrition and dietary needs.
• Reviews/revises the POC in coordination with the IDT and per regulatory requirements as participant status changes and as driven by needs identified as significant by the participant/family/caregiver.
• Develops and implements the nutritional components of participant plan of care including active participation and development of therapeutic dining programs.
• Monitors monthly weights, medical diagnosis, drug nutrient interactions, changes in chewing, swallowing, mood changes, hydration status, intake, skin breakdown, reports changes to the Interdisciplinary Team and makes recommendations for changes to appropriate staff.
• Oversees maintenance/cleanliness and orderliness of serving kitchen.
Monitors portion control; appropriate serving of food; adherence to therapeutic diets, purchasing and inventory of food, and local health code policies.
• Acts as liaison with contracted meal service to ensure meals meet the needs of the PACE participants.
• Responsible for the management of therapeutic diets, including modification to meet individual participant needs and physician orders.
• Documents nutrition notes and all other pertinent activities in the participant’s medical record according to the PACE standards in a timely manner.
• Communicates clearly, concisely, and in a timely and positive manner with team members, community partners, and participants/families/caregivers.
• Provides nutritional counseling and education for the PACE program participants and/or caregivers using appropriate materials.
• Provides nutritional educational programs to own department or other departments/PACE staff.
• Assists in monitoring meals, including intake, diet compliance, and tray accuracy.
• Maintains supplement/snack/bag lunch lists and dietary census for the PACE participants.
• Directs, supervises, performs initial and annual competencies on, and evaluates the performance of the food services staff members including contractors and vendors.
• Assists in development of policies and procedures for nutrition and dining services.
• Demonstrates commitment to 24/7 participant care by participating in on call, weekend, evening, and holiday on call rotations.
• Works with interdisciplinary teams to ensure effective, cost-efficient care.
Participates in center quality improvement activities.
• Other projects and duties as assigned.
Registered Dietitian Position Qualifications: • Currently licensed Registered Dietitian in the State of Kentucky.
• Minimum 1 year of experience working with a frail or elderly population or must receive appropriate training upon hire.
• Must maintain CPR certification.
• Must have flexibility to travel within the service area.
• Proficiency with technology-based tools, including electronic medical record.
• Able to work independently and as an effective member of a team.
• Excellent communication skills, with ability to communicate with internal stakeholders at all levels of the organization and external stakeholders from varied backgrounds.
• Excellent organizational skills; able to multitask and maintain numerous initiatives with time sensitivity.
• Working knowledge of principles and practices of healthcare, healthcare systems, managed care, Medicare and Medicaid benefits and regulations.
• Knowledge of safety and infection control requirements for healthcare facilities.
